Community Health Worker Program Interest Intake Form



Thank you for your interest! We are excited to offer this 7-week training program to provide individuals with the opportunity to become community health workers. A community health worker is a public health worker who is a trusted member of the community and reflects the values and culture of the community in which they serve. CHWs are the people on the front lines, providing advocacy, information and education to community members in order to help them improve their lifestyle and link them to their proper healthcare options.


Important Information: 

You are expressing interest in a full-time training program that will be offered Monday-Friday. Classroom hours will be from 9am – 5pm, apprenticeship hours will vary by site. 

The program is designed to help trainees gain the skills and knowledge to enter the health workforce and be successful as a community health worker. 

Trainees will receive a $1,500 stipend upon successful completion of the 7-week training program.

The entire first week of training will involve classroom instruction at a designated location in Nassau County. Each week, for the following 6 weeks, the training program will include one day in a classroom setting and four days in a community and/or health care setting as an apprentice. 

The training involves the use of a computer and you may be required to use a computer as part of your field-based learning. 

Although prior experience in health care is not required, trainees should have an interest in learning about medical conditions and working with healthcare professionals. 


Program Requirements:

Have ability to work in the US

Be at least 18 years of age by February 1, 2018

Have a high school diploma or GED (preferred)

Be able to successfully pass a post-offer background and health screening at Northwell Health Services and at the apprenticeship site

Be able to speak, read, write and understand English fluently. We welcome bi-lingual trainees!

Why would you be good in this role?

    Community health workers (CHWs) are the people at the frontline of the real social problems. Community health workers work in all facilities such as hospitals daycares, foster homes senior homes. The main role of  Community health workers is to help people advocate for themselves when individuals need to be heard because they feel as they have no  voice and are not socially accepted  People do not realize how  powerful a tool  an individual's voice is, even if  people have no other resources available to  them your voice is always your most  valuable resource that  no one can take  away from you. Community health workers need to help all citizens establish powerful voices so they are able to independently convey their own messages and communicate their ideas in a positive light by developing strong self advocacy skills. (CHWs ) help people solve problems by  fighting social injustice making communities a better place to live, therefore they develop a close understanding of the community they serve.  As a result, Community health workers create long lasting bonds in communities forming trusting relationships,  These trusting relationships enable them to serve as a liaison between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. For Example CHW can medicate between therapist and patient to build rapport of patients and their therapist . Community health workers are dedicated individuals who function along a field ranging from individual and community development to service delivery and promoting community empowerment and social justice. They often help link people to needed health care information and services. The community health worker serves as a bridge between the community and the health care, government and social service systems. Community health workers typically have deep roots or shared life experiences in the communities they serve. They share similar values, ethnic background and socio-economic status and usually the same language as the people they serve…


There are numerous reasons I want to be a  community health worker . I am a very open person talking about my problems with other people helps me alleviate my stress, find solutions to my problems, channel my frustrations in a positive way and acquire new perspectives  of other people. This will establish many connections with my clients. However,  I realize many people have  a difficult  time expressing themselves at all:  let alone opening about their  feelings to professional  strangers  is nerve-racking awkward judgments prone, and uncomfortable this Is  why  I believe  that without  the initial connections between client and therapists the therapies would never any progress because” the ice was never broken”  and trust does not form meaning therapy will not be effective connections are the  foundations  which create the necessary fire that allows people to build a good relationship  with their therapist  so they actually  feel comfortable  talking to a complete stranger and not feeling like there is something  wrong with them for going to therapy As a community health worker this training will help me gain  the skills to create an environment where people are not afraid to talk about religion or social problems. I plan to have open discussions with my clients, by asking open-ended questions .These conversations can help me break the ice and build a strong rapport with clients. A community health worker”s work cannot begin until a comfortable relationship with a client is established; a strong connection with clients is the foundation for therapy. I want to give individuals the opportunity to express what is going on without subjecting them to double-blinded question, in which however they answer, they are somehow wrong. Rather than put a negative connotation to their answer, I want to hear them out and allow them to express themselves more freely. As a result,  I plan to utilize this  training  so I can develop the skills to find  many unique  environments where all my clients no matter what struggles  circumstances  they have,  or background they are free to be who they want to be as a community health worker I can help design the best techniques to help my clients develop self advocacy skills to successfully use their voice to stand up for what they believe in as a therapist, I will teach my patients how to take advantage of their own abilities. As a result, my clients will learn to accept their differences and embrace their unique identities and talents, because no one should ever be ashamed of who they are. No matter what life throws at you, people can always achieve their dreams and goals. In order to progress as a society, we must cast away this false idea of what “normal” is. 


Helping others as a community health worker can enable me to gain experience in social work, which is one of my main goals and passions in life. I have always wanted to advocate for people because having cerebral palsy makes me realize how important it is to independently convey your own messages and communicate your ideas in a positive light. My experience with cerebral palsy has taught me that only the individual can truly advocate for themselves. As a result I have learned that even people with the best interests and intentions to help you (your parents, friends or your support system) can advocate for you. However, they will never be able to fully advocate for you, and convey your message, with the same level of passion, and importance that you have. Therefore, my disability enhances my desire to teach any individual self advocacy skills so they never struggle with expressing themselves.  As counselor, I can empower my clients with the ability to stand up for what they believe is right, and fight for justice. People do not realize how  powerful a tool  an individual's voice is, even if  people have no other  resources available to  them  your voice is  always  your  most  valuable  resource that  no one can ever  take  away from you . As a counselor I will make my clients utilize their powerful voices. I would like to become a counselor and make a difference in the world. By turning my individual situation of having cerebral palsy, into solutions that can create a path of advocacy for people who do not have a voice in society, As a community health worker, I believe I can enable my future clients to be confident in their own identity. Because I have a physical disability, my experience provides me with a unique sense of empathy for those who are disabled and whose opinions may not be heard. However, I intend to not only work with disabled individuals. I also plan to advocate for any individual seeking to discover the power of their own voice, which can be used as a tool enabling them to communicate their messages and experiences to make people aware of their problems. Because I have a physical disability, my experience provides me with a unique sense of empathy for those who are disabled and whose opinions may not be heard. However, I intend to not only work with disabled individuals. I also plan to advocate for any individual seeking to discover the power of their own voice, which can be used as a tool enabling them to communicate their messages and experiences to make people aware of their problems. Sometimes not being able to help people physically can be very challenging; however, I am very good at being empathetic and offering emotional support. This is what I experienced when my father was diagnosed with cancer. I could not do anything for him physically, but I sat home with him every day keeping him company; we watched basketball together, I talked to him, helped him with phone calls, and listened to him tell me stories. I encouraged him with positive words such as “be strong” and “I love you”. This experience made me realize that I want my future career to involve this individual care for people. As a counselor, I want to make people comfortable in their own skin, allowing them to use their voice to convey their messages and experiences. In addition, as a counselor I will provide emotional support to my patients by listening to their stories and having them take initiative to develop self-advocacy by leading discussions. The most important types of emotional support are patience and a positive attitude. As a counselor, I want to provide the most positive emotional support for all my patients. I will be there for my clients and provide them with hope and confidence in their stories, talents, and abilities, allowing them to develop self-advocacy skills and become productive members of society. My father taught me how powerful emotional support truly is, and providing emotional support to my patients will be more important than physical support.

 

I believe becoming a community health worker can provide me with the skills necessary to foster my development as a therapist. One who allows people to understand themselves so that they can reach their full potential and influence others to do the same. I want to enhance my skills as an empathetic advocate for individuals. I know how good it feels to have someone believe that you have the ability to make a difference and bring out your full potential; as a result, I want to enlighten people so that they embrace their differences and highlight the qualities that make them better people. I can achieve this by using my social skills and ability to fully attach myself to something I’m passionate about: the success of my patients.
